Youth GLIC Membership Application

Complete the application form below to register as a Youth GLIC member. A parent or legal guardian signature is required for all applicants under 18.

Eligibility
Open to youth participants who demonstrate an interest in golf, personal growth, leadership, and community involvement. Certain programs, events, or tournaments may include separate participation fees.
Conduct & Expectations
Members are expected to show respect toward instructors, staff, volunteers, and fellow students. Maintain a positive attitude, follow all safety guidelines, and represent GLIC with integrity, sportsmanship, and responsibility.
Youth Protection & Anti-Bullying
GLIC maintains a zero-tolerance policy for bullying, harassment, discrimination, or any form of physical, verbal, or online abuse. Any violation may result in immediate suspension or removal from the program. Members' safety is our highest priority.
Attendance & Commitment
Members are encouraged to attend scheduled practices, workshops, tournaments, and community service activities. Repeated unexcused absences may affect membership status. Parents/guardians must support program participation.
Media Release & Safety Waiver
GLIC may take photos or videos during activities for educational, promotional, or nonprofit documentation purposes. Members may opt out with written notice. By joining, members acknowledge inherent risks in golf and physical activities and agree to GLIC's liability waiver.
Refund & Termination Policy
All GLIC program fees, event registrations, and participant payments are non-refundable. GLIC reserves the right to terminate membership at any time for misconduct, lack of commitment, bullying, or any actions that negatively impact the organization. Membership is a privilege, not an entitlement.
